Your skin's firmness depends on two structural proteins: collagen and elastin. Collagen is the scaffolding that provides tensile strength and structure. Elastin is the spring that allows skin to snap back after movement. In your twenties, your body produces both abundantly. By your thirties, collagen production begins to decline. By perimenopause and menopause, the drop accelerates dramatically because estrogen, which directly stimulates collagen synthesis, falls sharply. Studies suggest women lose up to 30% of their skin collagen in the first five years after menopause.
This is not a cosmetic issue. It's a biological one.

What Is XERF?
XERF is a non-invasive structural skin tightening treatment developed by Cynosure Lutronic, a globally recognized leader in medical aesthetic technology. It received FDA clearance in August 2025, making it one of the newest and most rigorously validated skin tightening technologies available today.
What sets XERF apart from the sea of other radiofrequency devices on the market is something specific and significant: it is the world's first multifrequency monopolar RF system. The distinction matters, and we'll explain exactly why.
The Science: Multifrequency Monopolar RF
Most radiofrequency skin tightening devices work at a single depth. They deliver one frequency of energy, which means they can only heat one layer of tissue at a time. Effective for some things, but limited in scope.
The XERF system combines both 6.78 MHz and 2 MHz frequencies, allowing it to deliver energy not just deeper, but exactly where it's needed most.
Here's how those two frequencies work differently:
The 6.78 MHz frequency works in the upper dermis, the shallower layer closer to the skin's surface. This is where you see improvements in texture, fine lines, skin radiance, and overall skin quality. Think of it as the finishing layer.
The 2 MHz frequency travels deeper, reaching what's called the SMAS layer: the structural fibromuscular network that underlies the face and gives it its architecture. This is where real lifting and tightening happen. This is the layer that conventional RF devices simply don't reach.
As the tissue heats to controlled temperatures in its deeper layers, the body responds in two phases. First, existing collagen fibers contract, producing a subtle tightening effect that can often be observed immediately after the session. Second, fibroblast cells in the dermis are activated and begin producing new collagen and elastin in the weeks that follow.
This two-phase response is what makes XERF so compelling. You see something immediately, and then it keeps getting better.
Monopolar vs. Bipolar: Why It Matters
You may have seen RF treatments described as monopolar, bipolar, or multipolar. These terms describe how the electrical current moves through the tissue, and they have meaningful implications for depth and efficacy.
Monopolar radiofrequency works by placing one active electrode on the skin surface and a grounding pad elsewhere on the body, allowing electrical current to pass through tissue and generate controlled heat in the targeted areas. Because the current has to travel between two distant points, it penetrates much more deeply than bipolar RF, which keeps the energy between two closely spaced electrodes on the surface.
In practical terms: monopolar RF reaches the structural layers. Bipolar RF stays near the surface. For real tightening rather than temporary plumping, depth is everything.

Two more pieces of the XERF system deserve attention because they directly affect your experience and safety.

What Actually Happens to Your Collagen
This is the part most providers skip, and at Wholelistic Esthetics, we believe you deserve to understand exactly what is happening in your body when you receive a treatment.
The controlled thermal energy stimulates fibroblasts (the cells responsible for collagen production), triggering the release of growth factors that initiate the skin's natural healing response. This process encourages the production of new collagen and elastin, the structural proteins that keep skin firm, smooth, and youthful.
The histological evidence behind XERF is particularly compelling. In in-vivo studies, Cynosure Lutronic measured internal skin temperatures of 50°C, 60°C, and 70°C using ultrasound-guided fiber-optic imaging. Following XERF treatment, histological analysis confirmed collagen remodeling: specifically, increased thickening and tightening of collagen bundles at multiple tissue depths, along with an observable upward pull in the fascia.

A 2025 peer-reviewed pilot study published in the Journal of Cosmetic Dermatology followed 32 patients through 24 weeks of monopolar RF treatment. Significant improvements in jawline sagging and nasolabial folds were observed in 2D and 3D imaging. Quantitative analysis confirmed RF-induced skin rejuvenation, with a statistically significant increase in skin radiance at 4 and 8 weeks. Elasticity improved, with increased viscoelasticity and reduced retraction time.
The science is real, the results are measurable, and the improvements continue developing for months after treatment.
The Timeline: What to Expect
One of the things we love most about XERF is the honesty of its results. This is not a treatment that promises an overnight miracle. This is a treatment that works with your biology, and biology takes time to do its most meaningful work.
Here's what the actual timeline looks like:
Immediately after treatment: Many clients notice an immediate subtle tightening as existing collagen fibers contract in response to heat. Some providers demonstrate this by treating half the face first so clients can see the difference in the mirror before the second half is treated. The redness that follows typically resolves within a few hours.

XERF is not a one-and-done treatment. The collagen remodeling process takes time and requires commitment. A minimum of two sessions is recommended for meaningful results, with three sessions being our standard recommendation for most clients.
XERF is not a replacement for foundational skin health. This is a point we feel strongly about. XERF works best when the skin receiving it is healthy, well-nourished, and supported by a solid at-home protocol. That's why we always combine XERF with a Truly Lucent skincare assessment, because the structural work happens much more effectively in skin that has a healthy microbiome, good barrier function, and appropriate hydration.

During the winter months we usually are spending less time outside and experiencing less exposure to the sun on a daily basis. This makes the winter the perfect time of year to plan for spa treatments that may be a little more intensive. At Wholelistic we don’t use conventional chemical peels during our treatments but we do have a few types of appointments that help to speed up the results of your skin journey. Below you will find some details about 3 different treatments to help you decide if one might be right for you!
Microchanneling- This treatment utilizes a micro-needle device that creates tiny channels into the epidermis to encourage deeper penetration of a specific stem cell serum. This serum communicates with your skin’s cells as they regenerate and delivers specific information to encourage optimal renewal. There is no downtime associated with this treatment which makes it convenient to add to your busy calendar.
Who is it for? - Anyone interested in targeting hyperpigmentation from sun damage, acne scarring or melasma. Anyone that is concerned about skin laxity and fine lines. Anyone that wants a little boost to their already stellar skin care routine to continue to support the healthy production of the skins natural collagen.
Things to keep in mind- you will definitely be able to see glowy results from one microchannel session and the BEST results come from a series of three treatments spaced 4 weeks apart. Each treatment cost includes a Home Care Kit so you can feel confident that you are giving your freshly treated skin all of the support it needs for optimal results. The cost of one treatment is $400. Dermaplane can be added on for $30 and this year we are also offering a Lip Plumper add-on for $35. Plasma Fibroblast- This treatment utilizes a Plasma Arc device that uses electricity to create a precise lightning bolt of plasma energy that allows us to treat extremely specific areas. The arc creates superficial micro burns in a customized pattern to accomplish very precise skin tightening. In addition to the superficial effect, the plasma arc stimulates the fibroblasts located deep within your skin’s matrix. Those fibroblasts are responsible for creating new collagen! Visible results can be seen immediately after treatment but the full results can be seen three months after the treatment as it takes that long for the newly created collagen to move up to the surface of the tissue. There is some downtime associated with this treatment. You can expect to be able to see some swelling and superficial marks for 3-7 days depending on the size of the area treated and the speed at which your skin heals.
Who is it for? - Anyone that wants to target deeper wrinkles, scar tissue, skin laxity and fine lines. This is not a good treatment option if you tend to experience hyperpigmentation or have any conditions that cause the skin to heal slower than normal.
Things to keep in mind- This treatment is so specialized to each individual. A consultation is required so your esthetician can verify that this type of treatment will help best accomplish your skin goals. The cost per treatment varies based on the size of the area to be treated but treatment starts at $400. It may be possible to achieve your desired results with one treatment and it may be necessary to repeat the treatment in 3 months once the full results can be seen. Every appointment cost includes a Home Care Kit and 2 follow up check in appointments so your esthetician can monitor your progress and make recommendations on how to proceed.
Laurel Abundant Harvest Facial Experience- If those types of intensive treatments aren’t your jam, then you may want to consider this Abundant Harvest Facial. This is as close as we come to a conventional chemical peel style of exfoliation. The super potent whole plant products used in this treatment are all organic and are specifically chosen based on their healing properties as well as how abundant they were during that season’s harvest. This ensures that the products contain the highest amount of active properties as well as helps the small scale farmers utilize their bountiful harvest so nothing goes to waste!
Who is this for? - EVERYONE! This is a customizable facial that is perfect for everyone wanting a deeper ritual to nourish their skin during the sluggish winter months.
Things to keep in mind- Your esthetician will utilize multiple forms of plant based exfoliation to help encourage healthy cellular turnover so it’s best to avoid any other types of exfoliation the week before and after this treatment. We recommend that you drink plenty of water the day before and the day of the treatment because the lymphatic system is targeted during this treatment in a gentle but VERY effective way. Drinking enough water will support your system in flushing out cellular waste and toxins, giving you the best results of the treatment.

For combination skin, it’s important to use products that cater to each area. You can use a lightweight, oil-free moisturizer on oily areas and a richer, hydrating one on dry spots. This way, you are addressing the specific needs of each part of your skin.
Ingredients to avoid:
Alcohol can be too drying.
Fragrances can cause irritation and dryness.
Harsh exfoliants can strip natural oils and cause imbalance.
Comedogenic oils that can clog pores in oily areas.
Two of our favorites for combination skin are the Esse Nourish Moisturizer and the herbal skin solutions hydrating vitamin C serum.
What are the benefits of Gua sha?
Gua sha improves circulation, reduces puffiness, relieves tension, enhances product absorption, and promotes collagen production, lymphatic drainage, relaxation, improves skin elasticity, reduces inflammation.
